Information Security Policy Self-Check, FY2025

Thank you for your continued cooperation with our information security activities. The purpose of this survey is to have faculty and staff reconfirm key points regarding information security. It is required to be carried out every academic year under the university's information security policy.

There are21questions in total (or42questions if you are a server administrator). Related information is shown below each answer field, so please take this opportunity to review the setup of your own devices.

How to complete the self-check

1. Go to the web page below.

2. On the login screen, enter your Information and Media Center userIDand pass word.

3. Select “Yes” or “No” for each question. (Some questions ask you to choose from several options.)

4. When you have finished, click the “Submit” button.

If you are unable to access the survey page at all, you can download the question sheet and answer sheet from the page below and use those instead. Please return your answers by campus mail or email.
